VR HEADSET RENTAL RETURN AGREEMENT

This agreement outlines your responsibilities for the use and return of the virtual reality (VR) headset provided to you by Better Health Technologies, Inc. (“Better Health”) as part of your care program, in partnership with XRHealth.

1. Rental Terms:

You are being provided a VR headset to support your treatment through Better Health and XRHealth. The headset remains the property of XRHealth and is rented to you for the duration of your care program, up to 13 months.

2. Return Obligation

If you stop using the device before the end of your 13-month rental period, you must return the VR headset to XRHealth within 14 days.

If the headset is not returned within this timeframe, you will be billed a prorated amount for each remaining month in the rental period until the device is received back.

Your credit card will be held securely on file and charged only if the headset is not returned within the required period.

3. Device Condition

You agree to take reasonable care of the VR headset and return it in good working condition, including all accessories (e.g., charger, controllers, cables). You are responsible for any damage, loss, or failure to return the equipment.

4. Payment Authorization

By providing a credit card on file, you authorize Better Health to charge the card for any unreturned or damaged equipment per the terms above.

If you do not return the headset, you will be financially responsible for the remaining portion of the rental period fees, up to $2,300. No charges will be made if the headset is returned within 14 days and in acceptable condition.

If Medicare coverage for your VR headset rental ends and Better Health no longer accepts the assignment of benefits for this equipment, you may be billed directly for any unreturned or damaged headset as outlined above. Better Health will provide notice when Medicare coverage has ended and will document that this notice has been given.

5. Verbal Consent and Documentation

Your verbal consent to this Rental Return Agreement was collected and recorded during your EOAB process. We are sending you a copy of this agreement for your records.
